If any of these are missing or undecided, we cannot proceed with building your raffle or providing documents. You will need to complete a new intake form with all required information.

This form will not submit if any required fields are incomplete.

If you complete the form and do not see the "Thank You/Confirmation" page, it means some information is missing, and the form has not been submitted. You can go back and add the missing information without losing your progress.

If you close your window, your data will be saved automatically and will appear when you reopen the form.

Please note: Starter plans do not include raffles.
