This Claim Form, duly completed
True copy of Death Certificate or Confirmation Letter/medical documents by hospital, clinic, laboratory, or a competent authority, as the case may be (if relevant)
Copy of NRC card, passport, birth certificate, marriage certificate or any other relevant documents that prove the relationship between Life Insured and the Beneficiary(s).
Recommendation letter of specialist doctor(s) who treated (or) Ward-in-charge professor from which Life Insured was admitted.
Original medical bills from the respective clinic(s)/hospital(s)/laboratory(s)/dialysis centre(s) with the original seal(s) of the respective clinic(s)/hospital(s)/laboratory(s)/dialysis centre(s)/doctor(s).
NRC/Passport of Claimant
Any other supportive documents that may help in the processing of this particular claim
