Welcome to the Wisconsin Family Ties Referral Form

Who we support:
We partner with families raising children and youth with emotional, behavioral, or mental health challenges. Services are provided by trained Parent Peer Specialists who have firsthand experience navigating similar situations.

Who can make a referral:
Anyone who supports a child or family. This includes parents, caregivers, school staff, county workers, mental health providers, medical professionals, and other community members. 

What happens after you submit the form:
We will reach out to you to discuss the next steps and ensure you or the family you are referring are connected with the right support.
